Tags, Cancellation Reasons & Late Start Reasons
These are all pre-defined options to be picked from drop-down lists; this aids in speed of selection, keeps options easy to report on, and enhances data quality.
Missing Tags?
These need to be agreed by multiple users of THRIVE.
Please email support@informpeople.com to suggest any additions and reasons why they need adding.
When to use a tag?
- Tags should be applied to any list that is NOT a completely default list.
- Not all lists will require a tag and some tags may not be applicable at all locations.
- Any list with no tags would automatically be considered a core funded, 4 hour endoscopy service list.
Tags for Imaging
CDC Capacity
Lists that are being performed from a CDC funding stream or in a CDC unit
Training List
Use this tag for any training lists
Anaesthesia
Use this tag for any lists that include an anaesthetised patient or any list that requires an anaesthetist to be present.
Bank Holiday
Use this tag for any lists occurring on a bank holiday
Emergency
Use this tag for any unplanned emergency lists. This can include any lists starting at short notice
In Patient List
Use this tag for any list containing primarily inpatients
In Patient List – Partial
Use this tag for any list containing a min of outpatients and inpatients
Insourcing
Use this tag for any list performed by insourcing staff
IT Issues
Use this tag for any list that was impacted by any form of IT issues.
Nitrous Oxide Used
Use this tag for any lists where Nitrous Oxide has been used on the list. Please ensure staff are correctly listed when using this tag and use the notes to add additional staff in the room if necessary.
Off Unit Procedure
Use this tag for any list that requires staff to leave the endoscopy unit during a list and travel elsewhere, with or without equipment, to perform a procedure
Reduced Capacity
Use this tag for any lists that are planned shorter lists that do not fit the description of any of the above more specific tags (3.5 Hour, 3 Hour, Emergency, Inpatient)
Specialist
Use this tag for any lists performed by a specialist or containing specialist elements
Strike Action
Use this tag for any lists occurring during a period of strike action that may have been affected by them
Trial List
This tag can be used freely on any list trialling any new processes on a unit. This allows for data to be obtained quickly regarding any new processes to determine whether or not the new process is valuable
Hospital Cancellation Reasons - Imaging
Clinical Decision – Procedure Not Required
The list lead has made a decision that, despite the patient being booked in and attending the procedure, it is unnecessary
Complications with a Previous Patient
A previous patient’s session had serious complications, leading to the current procedure being unable to go ahead and therefore being cancelled
Equipment Failure
An important piece of equipment within the room failed, therefore the procedure was not able to be performed
Equipment Not Available
Equipment required for this procedure is not present and the procedure therefore can not be completed.
Medically Unsuitable
The patient was unable to undergo the procedure as they were deemed medically unsuitable by the list lead
Ran Out Of Time
The list time overran and the current planned procedure had to be cancelled due to a lack of time
Staff Issue
The procedure was unable to be performed due to a lack of staff, staff needing to leave before the list was completed or the procedure turned out to be too complex for staff involved to complete
No Blood Results
Could not complete the procedure, waiting on Blood results
No Translator
When the procedure has to be cancelled as there is no suitable translator available for the patient
Patient Discharged
The patient has been discharged by the hospital and is not present for the procedure.
Patient Cancellation Reasons - Imaging
Patient Cancelled Booking
When a Patient has contacted the unit in advance to cancel their procedure but at too short notice to rebook the slot
(Between the list being planned on THRIVE and the scheduled procedure start time)
Patient Consent Withdrawn
The patient chose to withdraw consent before or during the procedure
Patient Did Not Prepare
The patient did not correctly prepare for the procedure e.g. bowel prep, procedure specific medication etc
Patient Medication Issue
The patients current medication means they unable to undergo the procedure
Patient Unwell
The patient has become too unwell to undergo the procedure
Other Cancellation Reasons - Imaging
Did Not Attend
Patient did not attend for the procedure and gave no prior warning
Moved To Another Room
The procedure has to be moved to another room, where it will still be performed but must be removed from this list
NOTE: It is important to record and not delete these so they can be reviewed
Custom Reason
Historically an area for recording ‘other’ reasons, this cancellation now only occurs when a list is not closed correctly and procedures are left open indefinitely.
Late Start Reasons - Imaging
Booking Error
Error within the booking process leading to a late starting list
First Patient Wasn’t Present
The first patient on the list did not attend and therefore the list was started with the next available person on the list
Inpatient Delay
When an inpatient is late arriving to the list due to no fault of their own (often a delay in the porter system)
IT Issues
The list started late due to an IT Issue such as a booking system malfunction or connectivity issues preventing the start
No Blood Results
Could not complete the procedure, waiting on Blood results
Not Present – Anaesthetist
At the time the list was due to start everything was ready to go but there was no anaesthetist available to assist (when required)
Not Present – Nurse/HCA
At the time the list was due to start everything was ready to go but there were no nursing staff available to assist with the scope
Not Present – Radiologist
At the time the list was due to start everything was ready to go but there was no radiologist available to assist (when required)
Not Present – Leader
At the time the list was due to start everything was ready to go but the scopist was not present
Not Present – Support Worker
At the time the list was due to start everything was ready to go but there was no support worker available to assist (when required)
Not Present – Translator
At the time the list was due to start everything was ready to go but there was no translator available to assist (when required)
Patient didn’t consent
The first patient on the list didn’t consent to undertaking the procedure before the planned start time
Patient was late
The first patient on the list was late and caused the list to start late
Patient wasn’t prepared
The patient arrived on time BUT was not prepared in time for the start of the list
Previous List Overran
When the previous session went on too long and caused a delay in the proceeding session
Theatre was not ready
The theatre is available but was not prepared for use at the planned start time
Theatre wasn’t available
The theatre was in use by someone else OR for some reason the room is unable to be used for scoping purposes
Hospital Transport
Hospital transport was late and led to the list starting late.
Equipment Unavailable
There was no equipment available at the start time of the list meaning it started late.
